Photocard Licence Holders

You must complete a DL1 application form (available from most Post Offices, Local Vehicle Licensing Offices or directly from DVLNI by calling 0845 402 4000 and enclose a certified photograph with the appropriate fee. Send your completed application and fee to

Note: Photocard Holders – If your name has changed since your photocard licence was issued, you must also enclose original documentation confirming your new name. You must enclose a certified photograph. Send your completed application form, identity documents, photograph and fee to
